A 19-year-old female student presented to the university clinic following a hospital diagnosis of grade 1 fatty liver disease. The patient presents with grade 2 obesity, dyslipidemia, hyperuricemia, and elevated liver transaminases, compounded by a familial predisposition to obesity and type 2 diabetes mellitus. Given the patient's challenges regarding adherence to dietary and exercise recommendations, this evidence-based care report aims to investigate whether behavioral interventions, compared with standard care, more effectively facilitate clinical improvement in Metabolic Dysfunction-Associated Steatotic Liver Disease (MASLD) among young adults. A literature search was conducted via PubMed® and Scopus® using keywords and Medical Subject Headings (MeSH) on June 23, 2026. Three systematic reviews, three randomized controlled trials, and one cohort study relevant to the clinical inquiries were selected and appraised for their validity, significance, and applicability. Clinical outcomes across six studies consistently demonstrate that behavioral interventions outperform standard care in weight management and the reduction of liver enzymes. These results are further enhanced by eHealth interventions. Improvements in lipid profiles were reported but exhibited heterogeneity. One study with a three-year follow-up period indicated improvements in the hepatic steatosis index (HSI) and fatty liver index (FLI) as measured by imaging, suggesting that longitudinal follow-up may reveal structural clinical enhancements.
