Введение. Льготное лекарственное обеспечение является важным элементом социальной защиты, однако процедура его предоставления создает организационные издержки для системы здравоохранения. Посещения для выписки льготных рецептов составляют значимую долю в структуре обращений граждан в поликлиники, формируя дополнительную нагрузку на врачей первичного звена.Цель. Оценить влияние порядка выписки льготных рецептов на нагрузку врачей первичного звена и определяющие ее факторы.Материалы и методы. Проведен анализ нормативной правовой базы, регламентирующей льготное лекарственное обеспечение в Российской Федерации, в том числе в г. Москве. Изучены приказы Минздрава России, клинические рекомендации, распорядительные документы Правительства Москвы. Использованы данные о численности инвалидов и заболеваемости социально значимыми нозологиями за 2024—2025 гг.Результаты. Выявлено, что разрешенная длительность курса лечения при выписке льготного рецепта (180 дней) в два раза меньше, чем для пациентов без льгот (365 дней), что обусловливает более частые посещения поликлиник. Установлено противоречие между периодичностью диспансерных осмотров и необходимостью получения рецепта. Дефектура лекарственных препаратов выступает дополнительным фактором увеличения числа обращений. Численность инвалидов в Москве превысила 950 тыс. человек, зафиксирован рост онкологической заболеваемости (60338 новых случаев в 2024 г.).Заключение. Нормативные ограничения разрешенной длительности курса лечения при выписке льготных рецептов создают устойчивую административную нагрузку на первичное звено. Целесообразна гармонизация сроков для пациентов с хроническими заболеваниями и внедрение механизмов дистанционного продления терапии. Introduction. Preferential drug provision is an important element of social protection, but the procedure for its provision creates organizational costs for the healthcare system. Visits for prescribing preferential prescriptions constitute a significant share in the structure of citizens' visits to polyclinics, creating an additional burden on primary care physicians.Objective. To assess the impact of the procedure for prescribing preferential medications on the workload of primary care physicians and the factors determining it.Materials and methods. The analysis of the regulatory framework governing preferential drug provision in the Russian Federation, with emphasis on Moscow, was carried out. Orders of the Ministry of Health of the Russian Federation, clinical recommendations, and regulatory documents of the Moscow Government were studied. Data on the number of disabled people and the incidence of socially significant nosologies for 2024—2025 were used.Results. It was revealed that the permitted duration of treatment when prescribing a preferential prescription (180 days) is half that for non-preferential patients (365 days), which leads to more frequent visits to polyclinics. A contradiction was found between the frequency of follow-up examinations and the need to obtain a prescription. Shortages of medicines act as an additional factor increasing the number of visits. The number of disabled people in Moscow exceeded 950 thousand people, an increase in cancer incidence was recorded (60,338 new cases in 2024).Conclusion. Regulatory restrictions on the permitted duration of treatment when prescribing preferential prescriptions create a sustained administrative burden on primary care. It is advisable to harmonize the terms for patients with chronic diseases and introduce mechanisms for remote prolongation of therapy.
